You can apply in two ways:

• Online: Visit generatewealth.co.nz/managed-funds and follow the step-by-step instructions.

• Paper form: Complete the application form at the back of this Product Disclosure Statement (PDS).  

If you’re under 18, one parent must also sign the application form.

What you’ll need to get started

To complete your application, please have:

• Proof of identity (e.g., passport or driver licence)

• Proof of address (e.g., utility bill or bank statement)

• Your IRD number

• Your bank account details (for deposits and withdrawals)
